Start with one sentence

Write the main change you want the clinician to understand. Then add when you first noticed it, whether it is changing, and how it affects daily life.

Choose your top priorities

If you have ten concerns, choose the two or three you most want addressed first. You can still bring the full list, but priorities make it easier to use limited visit time well.

Bring useful context

  • A short symptom or cycle record, if relevant.
  • Your current medications, supplements, hormones, or sleep aids.
  • Recent medication or routine changes.
  • Questions you do not want to forget.

Before you leave

Write down the next step, what to keep tracking, when follow-up is appropriate, and any instructions about when to contact the office sooner.
